ARE YOU A CARE-GIVER?

New York City Caregiver Survey

The Alzheimer's Association, New York City Chapter is partnering with the Office of Manhattan Borough President, Scott M. Stringer to learn more about the impact that Alzheimer's disease has on New Yorkers. We have created a survey about caregivers' experiences accessing services for themselves and their loved ones. If you are a caregiver for someone with Alzheimer's or related dementia in New York City, please take a few minutes to take this survey which can be found at www.mbpo.org/caregiversurvey.

Hard copies of this survey can be found at the Chapter office located at 360 Lexington Avenue, 4th Floor, New York, NY 10017.

Alzheimer's disease has touched so many of our lives, and with a rapidly aging population, the number of people living with Alzheimer's disease in New York State is expected to rise by 30,000 over the next decade, reaching 350,000 by 2025. Our city must be prepared to respond to the needs of not only those with the disease, but those who care for them.

Our hope is that by learning more about the needs of caregivers and those living with Alzheimer's disease, we can take steps to ensure that New York is prepared for this growing public health challenge. We hope you'll help our fellow New Yorkers by participating in this survey.

Thank you.
